Only incident reports filed after January 1, 2007 are available for electronic transmission via this online request form.  You will need Adobe Acrobat Reader to open up the e-mailed report.  Social security numbers are redacted in accordance with Ohio Public Records laws.

You should wait at least four (4) days from the date the incident report was filed before submitting online report requests.  Requests will be responded to within a reasonable amount of time, Monday-Friday 8:00am - 4:00PM.
